As a member of Fort Family Health, same day appointments are our priority. our office maintains hours beyond the standard 9-5. Once joining membership, you will be given Jennifer's personal cell number in case of any emergency. This prevents unnecessary and Costly ER/urgent care Visits.

- Single Adult Membership >55yrs                                             $699.00/year

- Family Membership (from 2 – 6 individuals) <55yrs  *          $999.00/year (additional members will be $100.00 each)
 
- Family Membership (from 2 – 6 individuals) >55yrs  *          $1099.00/year (additional members will be $100.00 each)

*Family membership is defined as a married couple, a set of parents, or a single parent with dependent children under the age of 18 years old.
